What should I bring to the ER?
Health fund members
When arriving at the ER, you must present a referral letter from the doctor, a payment commitment from the health fund (Form 17) and an identifying document.
If you do not have a referral and commitment, you must obtain a commitment within 14 days of the treatment date. E-mail the form to: hovot@sheba.health.gov.il or call: +972-3-530-7475

Who is exempt of payment?
If you were hospitalized after visiting the ER, you are not required to provide a commitment.
Casualties of traffic accidents. The hospital charges the health fund (or IDF, in case of soldiers) directly and there is no need for a fund commitment or IDF budget reference. Unlike the past, you no longer have to submit a police confirmation.
In the following medical cases:
New fracture
Wound requiring suturing or fusion
Contractions/Labor
Babies under 2 months with fever over 38.5°
Foreign object in the eye
Shoulder/Elbow dislocation
Inhaling a foreign object
Magen David Adom evacuation from a public place or street following a sudden event.
Student injured during a school trip or activity, presenting a certificate from the school
Cystic Fibrosis patients
Cancer patients, where the ER visit is related to the disease
Traffic accident
Occupational injuries
If you are visiting the ER due to an work accident or traffic accident during work, obtain NII Form 250 and submit it to the health fund in order to get a commitment.
Soldiers
Soldiers referred by a medical physician, must present a budget reference. The form comprises an undertaking for the IDF to cover the cost of treatment. If you do not have a referral from a military physician, the treatment will be subject to a fee according to the Ministry of Health price list. A commitment may be submitted retrospectively (Form 119), providing the treatment date and signature of a military physician.
Without the required documents, the HaPoalim will be forced to charge you for the treatment.
