The program for healthy living with diabetes is designed to cater to individuals aged 60 and above, offering comprehensive care that takes into account the physical, mental, and cognitive aspects of the disease.
Led by experienced professionals specializing in diabetes treatment for individuals aged 60 and above, this program is the first of its kind in the country. Its goal is to enhance the quality of life for diabetes patients and their families by promoting health, physical resilience, proper functioning, and cognitive abilities such as thinking, memory, and learning.
Research indicates that diabetes accelerates the aging process, increasing the risk of conditions such as Type 2 diabetes-associated dementia and cognitive impairment. Additionally, there is a heightened risk of decreased muscle mass, limited mobility, dependency, and frailty.
The good news is that these risks can be minimized, and the course of the disease can be influenced. The key lies in evaluation, regular monitoring, and the development of a personalized, multidisciplinary treatment plan.

In the 360⁰ Diabetes program tailored for individuals aged 60 and above, our comprehensive initiative fosters a healthy life with diabetes through continuous monitoring of all facets of functioning. The program pioneers new advancements and boasts a team of seasoned experts specializing in the treatment of diabetes in older adults. It encompasses three distinct stages:
Stage 1: Assessment Day
A comprehensive day of medical, cognitive, physical, and nutritional assessment by a multi-disciplinary team specialized in diabetes. Concentrating these tests allows for their completion in a single day and location, eliminating the need for multiple appointments and travel arrangements.
Medical assessment: A thorough evaluation conducted by a diabetes specialist. The physician assesses the medical condition and determines the appropriateness of pharmacological treatment.
Cognitive and emotional assessment: A comprehensive cognitive assessment administered by a neuropsychologist, providing an overview of cognitive functions such as concentration, memory, executive functions, learning, and problem-solving. This assessment also includes an evaluation of the patient's mental state and quality of life.
Functional assessment: A physiotherapist examines and assesses various aspects of physical function in older age, including muscle strength, endurance, and balance.
Nutritional assessment: A nutritionist evaluates dietary habits and tailors them to meet the requirements of the treatment.
Stage 2: Development of a Personalized Diabetes Treatment Plan
Following the assessment day, the multidisciplinary team thoroughly evaluates the test results and formulates an individually tailored treatment plan. The plan encompasses treatment recommendations based on medical, cognitive, emotional, physical, and nutritional needs, along with suggestions for seamless integration into daily life. When necessary, patients are referred for additional tests and consultations with other specialists. Eligible patients are offered the opportunity to participate in the Case Management Program. Cost of Stage 1+2: Internal hospitalization Form 17, NIS 1,200.
Stage 3: Execution of the Diabetes Treatment Plan
The treatment plan is executed under the close supervision of the disease manager (case manager), who provides guidance and maintains regular communication, assists in scheduling appointments and coordinating among professionals, monitors various metrics, and serves as the primary point of contact for any questions or concerns during treatment.
