The Department of Medical Records and Information deals with the collection, preservation, oversight, and reproduction of medical records generated at the Sheba Medical Center, in accordance with the Patient's Rights Law.
You can obtain medical documents including:
- A copy of your disease summary or discharge letter
- Surgery reports
- Pathology results
- Laboratory and diagnostic tests
- Your full hospitalisation record
Who is eligible to receive information?
Patients can contact the medical records department via email, attaching a copy of their ID ((Teudat Zehut), to request information about their medical treatment. Family members seeking information on behalf of a patient are required to provide a letter from the patient that waives medical confidentiality in their favour. For minors, legal guardians must submit a request accompanied by a copy of their ID Certificate (Teudat Zehut) along with the appendix including the minor's details.
Requests from a treating physician should be sent with the clinic's logo, the patient's full name and ID number, a detailed description of the required information, the requesting doctor's stamp and signature, and along with the contact details for the requesting doctor, hospital or health fund involved.
To request information on a deceased patient, you can either email us or visit the records department with your ID card during our open hours. We will only provide the medical records to an immediate family member or to heirs listed in a probate order, a will execution order, or a court order.
What should the referral include?
- A copy of your ID card or passport. In the case of a minor - a photograph of the parent's ID card including the appendix with the minor's details.
- Full name
- Contact telephone number
Is there a fee for this service?
According to the Ministry of Health's guidelines, obtaining a copy of a medical record is subject to a fee.
Medical record from 2019 to present: 10 NIS.
Medical record before 2019: 124 NIS.
Summaries of hospitalisations, clinic visit summaries, laboratory test results, and imaging test results can be obtained, free of charge, in the personal medical file on Sheba Connect.
Obtaining medical information for ongoing treatment
To obtain information for further treatment, you should contact the treating physician who will submit a formal request. The information will be transferred directly to the doctor. Please specify in the request that you're not asking for a full hospitalisation record. No payment is involved for this information.
