Our unit provides support to patients facing the challenges and difficulties posed by diseases or injuries. These obstacles affect all aspects of their lives, impacting them on various levels: Emotional, social, interpersonal, family, and occupational levels. Treating the emotional reactions associated with a disability and their impact on family relationships can support physical rehabilitation and help patients maximize their time in the rehab department.
Our social workers are here for you and your family right from admission to discharge, offering emotional support (whether individually, as couples, families, or in groups) and helping you make the most of social entitlements. They act as mediators and liaisons with community services, carefully planning your discharge to guarantee ongoing care.
They also advise the multidisciplinary team in the department on psychosocial aspects of patient care.
Providing information and assistance in exercising your rights:
- Assistance in filling out claim forms for the National Insurance Institute (disability, special services, nursing law, claim for injury at work, mobility and more)
- Contact with the health bureaus for the purpose of arranging financial participation in the financing of rehabilitation equipment (wheelchair, toilet seat, cushion to prevent pressure sores, etc.)
- Liaising with insurance companies and lawyers for help in exercising the patient's rights
- Contact with RAM 2 and the Ministry of Defense
- Liaison with the social services departments and the health funds
Our social workers participate in departmental meetings, doctor visits and multi-team home visits aimed at providing recommendations for modifying housing to ensure optimal wheelchair accessibility.
Additionally, they handle the discharge planning in all its aspects and coordinate with the community to ensure the continued care for the hospitalized person even after discharge.
